Systematic preservation of the ossicular chain in cholesteatoma surgery using a fiber-guided laser.

چکیده
OBJECTIVES 1. To investigate whether systematic preservation of the intact ossicular chain in cholesteatoma surgery provides better hearing than dismantling the chain and reconstructing it. 2. To determine whether systematic preservation of the ossicular chain in cholesteatoma surgery can be performed safely. STUDY DESIGN Longitudinal parallel group study. SETTING District general hospital and private hospital. PATIENTS Adults and children who underwent primary intact canal wall cholesteatoma surgery. INTERVENTIONS Group A: Laser-assisted cholesteatoma surgery resulting in an intact ossicular chain. Group B: Laser-assisted cholesteatoma surgery resulting in a disrupted chain and an ossiculoplasty onto an intact stapes superstructure. MAIN OUTCOME MEASURES 1a. Conductive hearing loss (four frequency air-bone gap). 1b. Patient-orientated outcome (Belfast Rules of Thumb). 2a. Change in bone conduction threshold (four frequencies). 2b. Residual cholesteatoma rate. 2c. Rate of facial palsy. RESULTS There were eighty ears in Group A and sixty-nine in group B.The median hearing loss for Group A is 11.44 dB HL.The median hearing loss for Group B is 15.63 dB HL. (p = 0.001). Seventy out of eighty ears in Group A and forty-six out of sixty-nine ears in Group B fulfilled the Belfast Rule of Thumb. (p = 0.002). The null hypothesis is rejected for both of the hearing outcome measures. The two groups did not significantly differ in respect of any of the risk assessment measures. CONCLUSION The fibre-guided laser allows the cholesteatoma surgeon to preserve the ossicular chain in a systematic manner which is both safe and of benefit to the patient.

Ossicular Erosion in Patients Requiring Surgery for Cholesteatoma
Introduction: The aim of this study was to evaluate the condition of the ossicular chain in patients requiring surgery for cholesteatoma. Materials and Methods: In a retrospective analysis, the destruction of the individual and combined bony structures of the ear was described in 166 patients with cholesteatoma who went through surgery in our Otology Center between 2003 and 2009. Results:...
متن کاملStudy of Correlation of Pre-Operative Findings with Intra-Operative Ossicular Status in Patients with Chronic Otitis Media
Introduction: Chronic otitis media (COM) has been broadly classified into mucosal and squamous subtypes. COM types are associated with erosion of the ossicular chain. The aim of the present study was to correlate the type of COM, the site of perforation/retraction, and the type of disease pathology with the pattern and degree of ossicular chain necrosis. Materials and Methods: A prospecti...
متن کاملPredictive Factors of Recurrence in Pediatric Cholesteatoma Surgery
OBJECTIVE: This study aims to analyze factors influencing the recurrence of acquired pediatric cholesteatoma that has impact on operative plan decision. PATIENTS AND METHODS: Retrospective review of 62 children (68 ears) with acquired middle ear cholesteatoma who underwent surgery from 1995 to 2006. Age distribution at initial operation ranged from 3 to 16 years with mean age of 11.2 years. Sur...
متن کاملVestibular Schwannomas Microsurgery Assisted by Flexible Hand-Held 2 micro-Thulium-Fiber Laser
Background: Vestibular Schwannoma (VS) is one of the skull base tumors originating from vestibular portion of eighth cranial nerve. Recently, 2 micro-Thulium laser is used in the surgery of some intracranial tumors. Objectives: Assessing the efficacy of 2 micro-Thulium flexible hand-held laser fiber (RevolixjrÒ) in microsurgical removal of VS. Materials and Methods: This retrospect...
متن کاملHystology findings' correlation between the ossicular chain in the transoperative and cholesteatomas.
UNLABELLED Chronic otitis media is hystopathologycaly defined as the presence of irreversible inflammatory tissue changes in the middle ear. Ossicular lesions represent the most prevalent change. AIM to correlate the degree of ossicular chain changes seen during surgery with the inflammatory histological degree and the thickness of the cholesteatoma perimatrix. STUDY DESIGN Cross-sectional ...
